Reduce medical errors - always have an advocate

Medication errors are the number one reason patients readmit to the hospital. Often it's because the instructions given at the doctor's office or at time of discharge from the hospital are delivered directly to the patient. In so many cases, patients are overwhelmed by a diagnosis, a prognosis, the fast paced environment of health, medical jargon and so on that they just don't have the ability to absorb all the instructions given. In seniors with mild cognitive impairment or dementia, it's even worse. That's why having an advocate and/or recording those instructions are more important than ever.
